Details

Over the past year or so, the Ontario Medical Association has published policies on patient-centered care and chronic disease management as well as a background paper on health literacy. These interrelated topics have implications for the health care system, for patients and for practicing physicians. In this talk, Aura Hanna will discuss the system barriers to patient-centered care, particularly integration, access and funding, and the implications for physicians and patients of good communication, shared decision-making, health literacy and chronic disease management.
