Ontario Health Teams

Ontario Health Teams (OHTs) are being introduced to provide a new way of organizing and delivering care that is more connected to patients in their local communities.

Under OHTs, health care providers (including hospitals, doctors and home and community care providers) will work as one coordinated team – no matter where they provide care. We are developing a range of supports for OHTs including practice guides, measurement tools and an approach to evaluating Ontario Health Teams.

Community of Practice (CoP)

CoP hosts a meeting series on the second Tuesday of each month at 12 noon est.

This Community of Practice (CoP), facilitated by the Health System Performance Network (HSPN), is for members of all cohorts of Ontario Health Teams (OHTs). The CoP holds monthly virtual collaborative meetings on evaluation and performance measurement-related topics, creating a virtual learning-sharing experience with experts, OHTs, and academics. Members of this group have a dedicated space to exchange ideas and experiences. Members are also able to get access to resources, as well as share resources to advance their evaluation and performance improvement plans.

The Health System Performance Network has developed new surveys of patient and provider experience for the evaluation of Ontario Health Teams. HSPN aims to have a quadruple aim framework for evaluation and these are two critical dimensions of this evaluation.

FAQs

You should start with all providers and patients that you are seeking to be involved in implementing new models of care in your OHT and eventually expanding to all providers and patients involved in any aspect of your OHT.

You should start as soon as possible to get a baseline measure prior to implementing the new models. Expect to administer annually to check in and monitor changes in provider and patient experience.
