HSPN is looking for 25 community-based PRIMARY CARE Practices as pilot sites for this survey in Mississauga.

You (or a delegate in your practice) will distribute an electronic invitation to all patients AGED 18 and OLDER who you identify as receiving primary care from your practice.
We offer $500 for your participation as compensation to identify eligible patients and to distribute a link to the survey electronically. You need to have at least 300 patients aged 18 and older to whom you are able to distribute a study information letter and electronic link to the survey (our target is 75 responses per practice).
You will receive:
- Complete information about the study
- Email templates for distributing surveys to eligible patients
- A report providing anonymized results of your individual practice in comparison to all participating practices (only you will be appraised of your practice identity)
- Recommended practices for improving specific areas of your practice
- An opportunity to participate in CME/CPD-accredited training on the use of PREMs and PROMs in primary care
- Participation may also be eligible for Quality Improvement project requirements
The HSPN has undertaken a number of patient surveys in primary care and we are seeking an opportunity to enhance the use of these surveys in primary care. The primary purpose is to understand physician engagement in post-survey use of the results. This will be assessed during the post-survey educational event.
The survey data will be collected through a secure REDCap survey platform at the University of Toronto. Aggregate anonymized results will be shared with the individual participating practices and posted online on the HSPN website.
The University of Toronto researchers will also use this data—alongside data collected in parallel through Queen’s University—for research studies on factors associated with PREM and PROM outcomes. Research publications will also be shared through future notices to all participating primary care practices.
This initiative is supported by the Ontario SPOR Support Unit (OSSU) and has received REB approval from the University of Toronto. OSSU is funding this work because of their support for patient involvement in research (SPOR = Strategy for Patient-Oriented Research).
