Integrated Care

A primary focus of the HSPN is to understand, promote and evaluate integrated care efforts. Our work ranges from comparative summaries of integrated care programs around the world to evaluations of integrated care programs such as health links and the integrated funding model pilot program in Ontario. There is a compelling need to transform the health system by restructuring the provision of care to deliver integrated patient-centred care for individuals with complex care needs. Integrating the many care services provided by a diverse array of providers has been identified as a key pillar of a Canadian healthcare strategy.
