Evaluation Results and Reports

The HSPN Central Evaluation of the implementation of Ontario Health Teams (OHT) is designed to use both qualitative and quantitative data to assess 1) whether OHTs develop with the capability to achieve population-based, person-centred care (using a Formative Evaluation approach) and, 2) whether OHTs improve patient care and health system outcomes (using a Developmental Evaluation approach).  This page contains the Evaluation reports and publications emerging over the project, with the most recent appearing first.
